Dawn has the Following Types of Payment Plans:

·                     The Company Plan: $60 per Therapist hour. Set up fee $25 (Additional fees may apply for locations outside 30 mile radius of Lawrenceville, GA)

·                     The Employee Co-Payment Plan: Company pays a percentage of the $60 per Therapist hour cost. Set up fee $25 (Additional fees may apply for locations outside 30 mile radius of Lawrenceville, GA)

·                     The Employee Plan: On-site therapy paid by employee, minimum of 10 employees. Employee rate is $1/min.
